This patient with persistent hypertension after the removal of a left adrenal phaeochromocytoma had elevated levels of catecholamines in the right side of the neck on venous sampling. Subsequent carotid arteriography demonstrated a typical carotid body tumour. On this lateral film, the carotid bifurcation is seen to be splayed by the tumour which lies between the origins of the internal and external carotid arteries. The blood supply of the tumour arises from the proximal external carotid artery and a tumour blush is present.
